Professional Development Request
For any Professional Development request, always solicit approval from your building principal. If your request requires out of state travel or Title II funding, use the information below.
If you have a workshop you wish to attend that requires Title II Federal funds, please use the form above.
All requests must be reviewed and approved prior to entering into an agreement
Purchase Orders cannot be created without approval - Do not register or procure items prior to approval
For out of state requests, an out of state travel form must also be completed
Please complete this form well in advance of the date of the requested event/project (90 days is suggested)
A copy of the PO will be sent to the site if application is approved
If you have a workshop you wish to attend that DOES NOT require Title II Federal funds, please use the form above.
When complete submit the form to Beth Shope in the Superintendent's office for board approval
